Registration Form for the Administrator Course

October 10, 11, and 12 2018

8:00 a.m. until 5:00 p.m.

$899.00 for UALA Members (Facility that the person is working for must be members of UALA to receive this price)

$1299.00 for Non-UALA Members

This form and money must be turned in to by September 17, 2018 to reserve your space in the training.  Books are ordered by September 17 for participants to receive the books.  Please make arrangements for payment before that date.  Credit Cards are accepted.  Payment that is not received by the due date, your space will be released.  Each participant will receive a book. 

Mail registration and check to:  UALA c/o Corey Fairholm, 3042 W. Wilkins Peak Court, South Jordan, Utah 84095. The books will be mailed a week prior to the course.

The Training will be held at the Sagewood at Daybreak, 11289 Oakmond Rd, South  Jordan, Utah 84009, 385-351-9450.

The Utah Assisted Living Association has increased the cost of the class because the Senior Living University has updated their course with new books and more details in the course.  The association board decided that the State of Utah must be equivalent with other states in the administrator education.  The Bureau of Health Facility Licensing Certification and Resident Assessment has approved the new course.  A test will be given.  Once the course and the test are completed the required certificate will be given.  Another day has been added to the course.  The third day will cover the rules and regulations of assisted living in Utah, taught by Kristi Grimes, Bureau of Licensing, Certification and Resident Assessment. In addition, an Alzheimer’s required class will be taught on the last day following the rules and regulation class required for all administrators.  This will meet the required amount of Alzheimer’s training required by the rules and regulations. 

The administrator class will be a full three days, starting at 8:00 a.m. until 5:00 p.m. each day. 

Each participant must have their own set of books which is included with the cost of the class.

Emergency Preparedness and Risk Management:  What to do if you have a disaster in your facility.

Over the last year many catastrophic events changed the way that senior living facilities respond to disasters and accidents in their facilities.  Being prepared to take care of your residents and employees is key to your ability as facilities and operators to successfully come away from a disaster.  The purpose of this class is to help you and your staff plan for the aftermath of a disaster and how to put it all back together so our residents can return to their homes safe and sound.  Come ready to discuss how we can be safe in a disaster, make sure that it is paid for by your insurance partner and be ready to handle the uncertainties of different types of emergencies.  

Cost for UALA member is $25

Non- member is $40

Golf Tournament will be held on Friday, May 4.  Location TBA.  Prices for each golfer is $75

NAB CEU’s will be offered during this conference for three different classes including two in nursing and one in rules and regulations.  RSVP will be required to for the CEU’s. 

Other classes will include classes in activities, marketing, operations, nursing, and administration.

Utah Assisted Living Association Alzheimer’s Training in St. George, Utah

The cost is $100.00 per person.

This class will meet the requirements for the Alzheimer’s training according to the rules. 

A certificate of completion will be given to each student who completes the class.

The meeting will be held in the Auditorium in the State Office Building on the Northside of the Capitol Complex. Please park on the East Side of the Capitol.
